The Interstitial Lag

One of the most dangerous hurdles for patients relying on Continuous Glucose Monitors is understanding that CGMs do not measure blood sugar directly. They measure glucose in the interstitial fluid (the fluid surrounding your cells).

Because glucose takes time to diffuse from the capillaries into the interstitial fluid, your CGM reading is delayed by 5 to 15 minutes compared to your actual blood glucose. During rapid shifts (like exercising or treating a low), this lag causes dangerous over-corrections. Combating Diabetes Distress

Diabetes is as much a psychological burden as it is a physiological disease. The constant cognitive load of calculating insulin-to-carb ratios, managing unpredictable blood sugars, and responding to alarms leads to a recognized clinical condition known as Diabetes Distress.

When distress escalates into full burnout, patients often intentionally run their blood sugars high to avoid the immediate fear of hypoglycemia, causing their Time in Range to plummet.
